Navigating eldercare and disability is confusing…

But through virtual technology, good advice is both affordable and accessible to anyone needing guidance. My fees are clear, up front, and do not require a retainer or credit card kept on file. There is no lengthy contract, minimum hours, or administrative fees for simply being a client.

Are you needing immediate help understanding senior housing and costs? Do you want to maximize your safety and autonomy by educating yourself on what care is available when its time? Are you experiencing a decline and want to understand what aging with physical and/or cognitive decline might look like for you? Or are you recently disabled and need some guidance on life planning, costs, and care?

Do you have a Long Term Care Insurance (LTCi) policy and need a review and help understanding fine print or when to trigger your benefits? Do you need coaching on your upcoming LTCi assessment? Need help writing an appeal or advocacy? How about help communicating your wishes to your family, or formalizing a care plan for when you need care? Understanding options and making informed decisions are essential to aging well.

All you need for our consult is good internet, a laptop or other screened device, and possibly a friend or family to assist with a Zoom call. Convenient, flat-fee, timed sessions, are available as early as tomorrow. Use my contact page to request same day appointment.  I will fit you in if possible.

I have worked in four states since becoming a care manager in 1999 and serve clients/families all over the country. I’ve even served children in London and Saipan with parents in Minnesota and Virginia.

I’m looking forward to serving you!

We value our relationship and are always willing to work with you! You’re the best!

Services

One Hour Consult- 60 min.

$90

On our call, you provide an overview of your current health, concerns, resources, and how you’d like to see yourself as you age. I listen, ask questions. We work together on goals, solutions to your concerns, and options that make the most sense for your current/future needs and resources.

This call can also be utilized by families and friends who need an objective opinion or help focusing on the best outcome of a vulnerable adult in decline.

Our consult might also be booked by professionals with aged/disabled clients who are “stuck”, or have questions about costs, models, and options for their elderly clients who are planning for advanced age.

Parents of disabled adult children who will likely outlive them are also frequent clients for these calls. These consults help parents plan safe and healthy aging for their adult child, when they are no longer able to assist.

$399

Over the years I've reviewed many policies. They are all different as is the terminology and criteria for when and how to utilize benefits. Years ago, I taught CEUs for a John Hancock group in Minnesota on the importance of understanding the human component of these policies.

Years later, I frequently review and make recommendations on criteria for benefits, when to make a claim, how to make a claim, what supporting documentation to provide, and how to prepare for the insurance provider's initial assessment or phone interview.

This package includes (1 hour) virtual consult/assessment, and 3-4 hours of time reviewing physician’s notes, policy, and written summary with recommendations on maximizing your policy.

Consult with Record Review- 120 min.

$180

Consults include 60 minutes for records review prior to appointment (provided by you via email at least 24 hours prior to the appointment) and a 60 minute virtual call to go over results and how they might relate to your goals, care options, housing, and choices, as well as Q & A with care manager.

*Clinical records must be current and at least 6-12 months or more or physician’s notes, specialist notes including behavioral health, hospitalization history, and home care records, if applicable.

$349

Based on successful nationwide presentations I created for community groups and EAP programs.

Total 1.75 hours including tech prep and Q&A after 60 minute presentation on care options. Focus on care options for small and large groups over Zoom with Power Point. Group must manage technology for the presentation and have platform prepared with my Power Point (delivered by email to the purchasing group) prior to presentation.

Speaking Event on Senior Care Options

$90 per hour

Continued guidance, crisis intervention, care coordination, and advocacy, billed at an hourly rate of $90. Time can be purchased per quarter hour if desired.

****This option is only for clients who have already completed their first session with me and desire ongoing virtual support.

All appointments must be booked through this site. Calls may not be booked through email or by phone. If you prefer a phone call over a Zoom, you can use the Zoom link and dial in from a standard or cell phone. Video is required for all assessments/memory screens/clinical review/LTCi initial call.

Appeal Assistance for Rehab/Clinical Setting

$399

Appeal guidance for longer rehab and hospital stays require admission, discharge, and PT/OT/Speech summaries/notes to have a solid appeal for more care. I have had success assisting clients with these appeals. Start to finish, this service is about 4-5 hours of my time, including an hour Zoom call Q&A, and approximately 3-4 hours for me to review records and outline your appeal letter, prior to our Zoom.

Wording and approach is important. You will be required to provide all previous and current appeal attempts and letters from providers and insurance in addition to clinical notes and history. There is no guarantee for approval, however, you will have a better understanding of how to use documented clinical facts to argue a reasonable discharge date.

Memory Screening with Written Report- 120 min.

$249

Memory screening consult call begins with a 30 minute discussion of health history, a brief cognitive assessments (about 30 minutes or as tolerated), and concludes with 30 minute Q & A with care manager for concerns, care options, and costs. This must be done over Zoom, not a phone call.

This consult also provides a summary (delivered by email) of findings within three business days to be shared with your primary doctor and family, spouse, partner, or trusted friend.
