Serving clients and families remotely in Maryland and out of state with consulting, advocacy, assessments, care planning, care coordination, and crisis guidance. Appeals for Medicaid, rehab/Medicare, waiver guidance and training, assistance in hearings for vulnerable adult clients, memory testing, placement to senior housing and clinical settings, speaking to large and small groups on age and disability-related topics, and assisting other clinicians with complex cases.
Independent Contractor- Aging Life Care Manager/Gerontologist, May 2016- February 2020
*Perform all aspects of care management for largest care management practice in Baltimore, including comprehensive assessments, care planning, crisis intervention, advocacy, placement, and education to support adult clients, their families, spouses, partners, and friends.
*Build multidisciplinary care teams to combat barriers to optimal health for vulnerable adults navigating the health care arena. Clients include the aged and adults suffering from mental health issues, TBI, MS, DD, degenerative cognitive deficit and/or age-related disease.
*Market and network to educate the community and other professionals on issues related to gerontology and care management.
* Speak by invitation at senior housing facilities and law firms on aging transitions and support services. Large group webinars for employee benefits/education. Recent presentations include senior housing choices and care options for Fannie Mae employees.
*Advocate for Maryland State laws that foster autonomy and patient-directed care for the aged and disabled adults at the state level by serving on a panel for home and community-based waivers (2019, 2020).
*Design and implement monthly case review initiative for largest care management practice in Baltimore.
*Create online working template for team to update client status and schedule/facilitate monthly conference call for approximately fifteen clinicians.
*Review and edit report and care plans of coworker clinicians. Assist with training and shadowing opportunities for newly hired clinicians.
*Screen potential new hires and conduct phone and in-person interviews. Review writing samples and resumes. Make recommendations for new hires to the president based on those meetings.
*Update policies and procedures as appropriate to assist in maintaining consistent and high caliber service offerings and compliance with ethics established by ALCA.
Estates in Transition, Inc. “Rent A Daughter”, Twin Cities, MN

*Established new client accounts through marketing efforts, public relations, speaking engagements, and community education.
*Provided geriatric care management services including assessments, care plans, and on-going services to clients.
*Increased client base by 25% during first year as director.
*Created consulting division of the company, products, and related literature as well as landing the first consulting project from a home care agency with eight offices in the Twin Cities, and 400 nationwide.
*Developed an Employee Assistance Program (EAP) offering companies a la carte eldercare services for employees to purchase each month to combat absenteeism due to eldercare issues.
*Published article entitled “Geriatric Care Managers: Professionals to Help Navigate the Eldercare Arena” in Minnesota Healthcare News, January 2004.

*Featured Gerontologist on “Strictly Seniors with Roland Minda”, a cable television program in the Twin Cities.

*Increased client base by 10% through marketing; generated $9,000-$11,000 revenue per month through billable hours of care management from 10/99-12/99.
*Managed caseload of 14-22 clients (nearly half of all ACM accounts).
*Performed duties of a Geriatric Care Manager; represented ACM at networking events, health fairs, professional meetings and educational seminars.
*Evaluated intensive and complex cases including service to clients with developmental and psychiatric disabilities, memory impairments, chronic illness and extreme family dynamics.
